How Do I Request a Lab Test As A New Patient?

If you need convenient online access to a Healthcare Provider you've come to the right place! When you register as a patient on Push Health, you're matched with a Healthcare provider licensed in your state. They will consult with you to offer medical advice and review all of your lab testing and prescription requests. 

You can simultaneously fulfill your patient registration and submit your first request by filling out a  'Request Care' form at the link below.

The Request Care page will look as shown below. The column on the right lists some of our popular lab tests offered. By clicking on one of the test links, this will submit your lab request and simultaneously register you as a patient on the Push Health platform. 

If you don't see a test listed, click on the blue  'Lab Testing' tab displayed at the top.

Once you click on 'Lab Testing' it takes you to a page that looks like this. Here you can enter the name of a lab test to verify if we offer it on the Push Health platform.

For this example, we added the Lyme Disease Test to our cart. After adding the test, you will choose a lab location by inserting your zip code. It will list the closest Quest Diagnostics laboratories in your area. Unfortunately, Push Health doesn't have a corporate tie up with every Quest lab location. The search results won't display lab locations in certain areas of NY and all of Arizona state.


In Step 2 and 3, you will enter your Patient Demographic and Health Information. If there are any health questions you don't know how to answer you can enter "unsure."

In Step 4, the total fees are outlined for you to review. The cost is all-inclusive. You won't need to pay again when visiting the lab.

The total fees cover:
  • Your medical consultation with a Healthcare Provider
  • Push Health platform fees
  • A Lab order issued by your Healthcare Provider
  • All Lab testing fees
  • and Test results

Click on the green  'Submit Request' tab at the bottom of the page to proceed to the next step.

After clicking on 'Submit Request' you will receive a pop up like this.  Please review the details entered, to ensure you have provided correct information. Typos and errors can cause delays. If something needs to be fixed, click on the red 'Edit' tab in the bottom left corner. If everything looks good, click on the green 'Submit Request' in the bottom right corner to proceed to the next step.

The last and final step is providing your payment details. You can enter in your debit or credit card details by clicking on 'Card' 

Or you can make a payment through your Paypal account by clicking on 'Paypal'

All payments are processed with  Braintree. They are owned by PayPal and specialize in processing secure mobile and web payments for e-commerce companies. For your safety and security the payment details are never stored on the Push Health platform.

After entering your payment details the system will prompt you to set up a password so you can login to your Push Health account. You will receive a request number and a confirmation notification by email. 

Once you have completed these steps for submitting a lab request, the system will begin the process of matching you with a provider that is licensed in your state. When you are connected, the provider will review your request. 

Once it's been approved a lab order will be issued that looks like the example shown below. You will be able to either download the PDF to your smart phone or print and carry it with you to your lab visit. Taking the lab order with you will ensure that results are posted under our corporate Push Health account,  so we can promptly deliver results as soon as they are available. 

Before visiting the lab you will first need to confirm if they accept walk-ins. Most Quest Diagnostics laboratories now require you to schedule an appointment beforehand. You can conveniently schedule an appointment on their website. Click on the link below and select ' Schedule Appointment' to get started.
