NPDB Self-Query Requirements and Help

To activate any pending licensure, please upload a current National Provider Data Bank (NPDB) Self-Query to your Push Health profile that meets the following requirements.


Push Health NPDB Self-Query requirements:

  • Any licenses you wish to activate on your Push Health account must be present on the NPDB Self-Query you upload (any licenses not included, will not be activated) 
  • NPDB Process Date must be within 3 months of uploading.
  • NPDB Self-Query upload must include its digital certification from the NDPB website (see below for additional information)

The Google Chrome web browser is recommended when performing your self-query. Other web browsers may create errors which can lead to delays and frustration.

How to ensure your NPDB includes its digital certification:

To include the digital certification, please download your self-query results using the icon we have circled in our example image below:

When you upload your NPDB Self-Query to Push Health, you may receive the following message: “ERROR: Upload invalid, digital certificate failed. New file required.” This means the version of your NPDB Self-Query you uploaded, did not include its digital certification.


If you downloaded your NPDB Self-Query, here is a test you can run to see if it includes its digital certification:

  1. Please download Adobe Acrobat Viewer: Download Free Version Here
  2. Download your Self-Query using the blue "Save" button
  3. Open Adobe Acrobat on your computer > Click "Menu" in the top left corner and select "Open" from the drop-down menu
  4. Find and select your Self-Query report in the file explorer window
  5. View your Self-Query within Adobe Acrobat. If the digital certification is included you will see the following message across the top of your document (outlined in red):


If you do not see this message when you view your Self-Query within Adobe Acrobat, it means your file does not include its digital certification. We recommend the following steps:

  1. Ensure that you use Google Chrome to download your file
  2. Ensure that you downloaded the file using the icon circled in red (first image above)
  3. Contact the NPDB Help Center and explain the issue you are experiencing and the steps you have taken to resolve it

If the message does appear for you, then you are good to go. You can upload the file as-is and continue with your account onboarding.
