Can I Obtain A Doctor's Note?

The Push Health platform is primarily set up to provide patients with basic prescription and lab testing. When you submit your request you will be matched with a Healthcare provider that is licensed in your state. Anything additional such as a doctor's note would be at the discretion of the provider. 

Each provider is different so they may approve or decline to offer assistance with this. There is no way for us to guarantee that a doctor's note would be provided.

If you haven't yet registered on the Push Health platform you can do so by filling out a 'Request Care' form at the link below. This step will begin the process of connecting you with a Healthcare Provider.

If you have already registered you can feel free to confirm this directly with your provider. Please login to your account dashboard with your email and password at the link below. Select your patient profile and click on 'Send Message' to connect with your provider directly.

 If the Healthcare provider agrees to provide you one they may submit an invoice to charge you for the additional service. The invoice would be a separate charge in addition to the initial request fee.
