How Do I Send My Photo ID To My Provider?

We have made it simple to upload an image of your photo ID to your account. Follow the easy steps outlined below.


After signing in and selecting your name, you should see a screen that look similar to this:

Next, you'll click on the My Profile button to bring up your personal information:

Scroll down to the bottom of the Personal Information section. You will see a button to "Upload Image of ID".

After clicking on 'Upload Image of ID' you will be able to take a picture (when on a mobile device) or attach a .jpg, or .png file from your device. And that's it! Your Push Health profile is updated with your ID!


Uploading ID FAQs:

  1. I upload an image of my ID but I get an error and the "Upload Image of ID" button does not disappear? This means you have uploaded an invalid file type. You can only upload a .jpg or .png file type.
  2. The "Upload Image of ID" button is not there? This means you have successfully uploaded an image of your ID.
  3. Have a new ID you need to upload? Email us at help@pushhealth.com from the email linked to your Push Health account and let us know you need to upload a new ID.
